Lesson 5: The Hallucination Problem in Pipelines — When AI Gets It Wrong at Scale

The Difference Between One Mistake and One Hundred

You're using ChatGPT. You ask it to look up the release date of a movie. It gives you a date. You notice it's wrong. You correct it. You move on.

One mistake. You caught it. No harm.

Now imagine your workflow runs an AI step 100 times a day. The AI hallucinated on day 5. But you didn't notice. It ran 50 more times before you caught it on day 12. You now have 50 pieces of bad data in your system.

That's the hallucination problem in automation: errors compound. The AI isn't there to be corrected in real-time. It runs unattended. And wrong output gets treated as truth.
